Addison County Home Health & Hospice, Inc was founded in 1968 by a group of Addison County residents for the purpose of providing quality health care, patient education, and social services to individuals in home, group, and institutional settings. Governed by a volunteer board of directors, Home Health is a not for profit agency offering comprehensive home care services in Addison County.
Home Health provides skilled health care services consisting of nursing, physical therapy, speech therapy, occupational therapy, medical social work and nutrition consultation. Home Health also provides support services of home health aides and homemakers for patients in their homes.
